Aging is inevitable. It doesn’t have to be unpredictable.
Care begins with listening. — Dr. Yaw
Meet Your Guide
Hi, I'm Dr. Yaw.
I'm a geriatrician and the founder of Y & M Health Solutions. For nearly two decades, I've helped older adults and their families make sense of what's changing and what to do about it.
I don't believe in generic answers. I listen first, explain things in plain words, and help you find the next right step for you and your family. And when I don't have the answer, I'll help you find the better or best one.
Because care begins with listening.
Welcome to the Aging Navigation Circle
The Aging Navigation Circle is a warm monthly gathering led by Dr. Yaw, a geriatrician who has spent nearly two decades helping older adults and their families make sense of what's changing.
The Circle is for you if you're growing older and want to stay independent. It's also for you if you're caring for a parent, a spouse or someone you love and wondering what's normal, what isn't and what to do next.
Each month, Dr. Yaw sits down with the Circle and explains aging in plain words: memory changes, caregiving, doctor visits and planning ahead. You'll learn what to watch for, what to ask your loved one's doctor and how to take the next step with clarity instead of panic.
Think of it as time with the doctor next door. There's no rush and no confusing medical talk. You get real answers, and you hear from families walking the same road you are. Because many hands make caregiving easier, the whole Circle learns together.
Ask questions. Get real answers. Plan ahead together.

Help for Grammy
By Nana Yaw Adu-Sarkodie · Illustrated by Sunil Francis
Help for Grammy gently explores how a child experiences and understands a loved one's dementia. As Jaylen notices changes in Grammy, they are encouraged to ask questions and share their feelings. Through honesty, love and memory, dementia becomes less frightening and more manageable, showing children that care, connection and cherished moments still matter, even during change.
